|
It was reported that doctor failed to sink screw, impacted liner and damaged the liner.It was promptly removed from the patient.After inspection of the acetabulum for debris, a new liner was opened and impacted after screw was seated with screw driver.Second liner was seated.First liner was explanted.There was a 5min of surgical delay was reported.

Additional information was received and stated that the impression was made into the plastic liner.Deemed imperfect.There were no reports of patient harm and nothing was left in the patient.
