Event Description

According to the reporter, during a cholecystectomy, when positioning the device in the abdominal cavity, the doctor pressed the handle to release the clip, however instead of firing the clip, the device made a noise and the clip did not come out.Few attempts were made to fire outside the patient cavity without success.The part where the clips goes did not close.The surgical time was delayed 10 minutes due to the issue.
