Event Description
|
According to the reporter, during bilateral salpingo-oophorectomy, some activations ended up with poor seal and bleeding (activations reached to the end and the energy was running through the jaws visible).Later, towards the end of procedure, all activation attempts reached to "incomplete seal cycles", at least 10 activations.Blood transfusion was not necessary due to bleeding.Jaws of the ligasure handpiece were cleaned with normal frequency.About 10 good seals were completed before the problem occurred. The tissue bundle had normal thickness, not too thick or a type that has not regularly being sealed when the problem occurred.Another ligasure was used with the same generator and it worked fine. The procedure was completed by traditional bipolar and scissors.
