The device was returned to olympus for evaluation and the evaluation found the reported malfunction could not be confirmed.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
It was reported that the high flow insufflation unit could not insufflate during a tlh (total laparoscopic hysterectomy).The equipment was not set correctly, and the surgeons had a hard time visualizing the operation field due to too much smoke and was challenging to conduct the operation.However, the procedure was completed after a delay of 30 minutes while the patient was under general anesthesia.The whole operating room and hallway smelled of polluting smoke, there were no reports that any emergency/urgent issues/adverse conditions arose that required medical intervention or treatment.There is no evidence that a life-threatening event occurred, or that the patient or heath care staff developed a permanent disability or permanent damage that caused substantial disruption in their ability to conduct normal life functions, or that additional intervention was required to preclude permanent impairment of a body function or damage to a body structure.Though there was a delay this delay did not cause harm/injury/adverse clinical impact.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a root cause could not be determined.Olympus will continue to monitor field performance for this device.
