Event Description
|
It was reported that the patient presented for an implant procedure.During the procedure, it was noted that the catheter failed to advance in the left ventricular lead.While maneuvering, the left ventricular lead was dislodged, and the left ventricular lead was not used.The physician decided to proceed with the procedure with the bipolar device, and the patient was discharged post-procedure.

The reported events were lead dislodgement, failure to advance in catheter, and unable to implant.As received a complete lead was returned in one piece for analysis.The event of failure to advance the lead in the catheter was not confirmed.Visual and x-ray examinations of the lead found no anomalies.The lead was able to be inserted and advanced through the catheter with no difficulty.Electrical testing did not find any indication of conductor fractures or internal shorts.
