Event Description
The customer reported to olympus that the endoscope reprocessor was used to clean a scope.When the scope was removed, white silicone foreign material was found inside the washer.The scope was cleaned with another reprocessor; no foreign material was found after the second reprocessing.No patient involvement was reported.

The device was returned for evaluation.During examination, the reported issue was confirmed: foreign material was found in the washer.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the reported white foreign material inside the washer was determined to be an organic silicone.Some silicone-based sealants are used in the connectors that connect the cleaning tube of the cleaning machine and in the assembly work of the internal unit, however, due to the structure of the product, there is no possibility of the sealant leaking out or entering the liquid delivery pipe, and it was also confirmed that any abnormalities can be detected during inspections during the assembly process.The root cause of the foreign material could not be determined.Olympus will continue to monitor field performance for this device.
