Event Description
It was reported that during a procedure, where a set meniscus mender ii was used, it was not possible to pass the suture through the needle.Surgery continued without any delay with a back-up device.No further complications were reported.As per investigation results the suture retriever was not functional due to one snare being fractured from the hub and both snare loops being deformed.

The reported device was received for evaluation.A visual inspection of the returned device found that it is not in its original packaging.Both suture retrievers have the snare deformed and one has been fractured from the hub.The shaft of one of the needles and one of the suture retrievers are bent.A functional assessment of the device found the suture retriever not functional due to one snare being fractured from the hub and both snare loops being deformed.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specification upon release for distribution.A complaint history review found similar reported events.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate.The root cause has been associated with unintended use of the device.Factors that could have contributed to the reported event include an application of unintended inappropriate, excessive force to the device or an inadvertent impact event inconsistent with normal use.No containment or corrective actions are recommended at this time.
