Event Description
A physician was using an amphirion deep pta balloon catheter for treatment of a 200mm calcified plaque lesion with 50% stenosis in the proximal region of the left peroneal artery.The artery was 3mm in diameter and mildly calcified with no tortuosity.A 6fr non medtronic sheath was used and a 0.014" non medtronic guidewire.A non medtronic inflation device was used with iodixanol inflation fluid.No damage was noted to packaging.The device was prepped as per the ifu with no issues identified.The device did not pass through a previously deployed stent.No resistance was encountered during advancement.When the device entered the patients body for the first time the balloon expanded and deflated normally.It was reported during the second inflation blood was observed coming out of the pressure pump.Under digital subtraction angiography (dsa), it was found that the balloon was broken and could not be deflated.Then, with the balloon open, it was withdrawn from the sheath outside the body.The balloon was tested in vitro and it was found that at 10 atm a circumferential/radial burst had occurred at the proximal end of the balloon.The balloon was replaced with the same model and the surgery was completed.
