It was reported that during use of the cardiac resynchronization therapy defibrillator (crt-d) the clinician noted the patient heart rhythm in the twenties and thirties from paramedic¿s external cardiac monitor, which is below the crt-d programmed lower rate setting and noted as bradycardia. however, the clinician was unable to confirm on hospital external cardiac monitor.The current rhythm electrocardiogram (ecg) showed heart rate atrial sensed - ventricle paced 93 beats per minute. possible inappropriate therapy noted for supra ventricular tachycardia (svt) versus ventricular fibrillation (vf). the crt-d remains in use. it was also reported that the left ventricular (lv) lead exhibited high thresholds. the lv lead remains in use. no further patient complications have been reported as a result of this event.

It was further reported that the patient did have a device check after the reported event.The shock was appropriate and the device pacing below the programmed rate was not confirmed.The patient¿s thresholds are variable on the lv lead.No reprogramming or intervention was done.
