Event Description
According to the reporter, during the mediastinal tumor procedure, the device presented faults: when triggering the gripper, it appreciated information that it was not possible to complete the cycle.The jaws of the forceps were cleaned with wet gauze but continued to not work.Switched off the equipment and the problem persisted.Swapped the equipment for an ls10, but it did not work.There was no bleeding after cutting, the vessel was fully transected, and there was no bleeding observed directly from the seal.Replaced it with another ligasure using generator with software version of 4.0 and it operated normally.There was no patient injury.

Event Description
According to the reporter, during the mediastinal tumor procedure, the device presented faults, when triggering the gripper, it appreciated information that it was not possible to complete the cycle.Re-grasp alert, other alarm or an error occurred.Activation and end tone were heard but sealing was inadequate/partial.The jaws of the forceps were cleaned with wet gauze but continued to not work.Switched off the equipment and the problem persisted.Swapped the equipment for an ls10, but it did not work.There was no bleeding after cutting, the vessel was fully transected, and there was no bleeding observed directly from the seal.Just at the moment it developed a defect when cleaning of the jaws of the device's handpiece was performed.Tissues and vessels in the lung region were being sealed when problem occurred.The thickness of the tissue band and vessel was less than 7mm, they were approximately 4mm thick.Replaced it with another device using generator with software version of 4.0 and it operated normally.There was no patient injury.
