Event Description
|
It was reported that a patient presented with grade 4 functional mitral regurgitation (mr) for a mitraclip procedure.During transseptal access, the blood pressure dropped to 60mmhg and a cardiac tamponade occurred.Surgical drainage was performed.The entry was from the right atrium, and it the cardiac tamponade was treated.The procedure continued after the patient was stabilized.One clip was placed on anterior 2 and posterior 2 leaflets.The mr was reduced to grade <1.After withdrawing the steerable guide catheter (sgc) a hole was found at the fossa ovalis.No additional information was provided.

The device was not returned for evaluation.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot.Based on the information reviewed, the reported perforation appears to be related to procedural conditions.Perforation is listed in the instructions for use (ifu) as a known possible complication associated with mitraclip procedures.Serious injury/illness/impairment was a result of case-specific circumstances.There is no indication of a product issue with respect to manufacture, design or labeling.
