Event Description
The customer reported to olympus, the choledocho videoscope distal end had air/water leaks when not in use.The issue was found at preparation for use in a diagnostic procedure.The procedure was completed using a similar device.There were no reports of patient harm.The device was returned and during the device evaluation the following reportable malfunction was found: the angle could not be maneuvered due to a broken angle wire.The medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.
 
Manufacturer Narrative
The device was evaluated by olympus and in addition to the findings in the b5, the following malfunctions were found: broken channel tube resulting in no suction, and water tightness loss; connecting tube was dented and wrinkled.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.New information added to the following fields: h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over one (1) year since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be determined.However, it is likely that angulation became locked due to physical stress from forcible angulation with the bending section angulated, or angulation with excessive force.Olympus will continue to monitor field performance for this device.
