Event Description
According to the reporter, during a laparoscopic assisted vaginal hysterectomy (lavh), the device¿s jaws would not open up and the knife blade trigger locked.The issue was after normal functioning of the device for half the procedure.Another device used successfully with the same generator.There was no patient injury.

According to the reporter, during a laparoscopic assisted vaginal hysterectomy (lavh), the device¿s jaws would not open up and the knife blade trigger locked when applied to tissue and removed after cut dissection.The issue was after normal functioning of the device for half the procedure.Another ligasure device used successfully with the same generator.There was no patient injury.
