Event Description
The customer reported to olympus, there was too much pressure in the air water channel of the evis exera ii ultrasound gastrovideoscope during automated endoscope reprocessing treatment.The device was returned for evaluation.During the device evaluation, whitish material was found clogging the nozzle.There were no reports of patient harm.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.
 
Manufacturer Narrative
The device was returned to olympus for evaluation and the evaluation found too much pressure in the air-water channel during aer treatment, bending section cover glue is separated, scratching marks on the key top 1 rubber and the acoustic lens surface, and the nut on the elevator channel plug is damaged.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the observed foreign material in the nozzle appeared to be a solid, whitish material, similar to cleaning agent residue but otherwise could not be identified.A definitive root cause of the issue could not be determined, as there was no deformation observed that might result in the retention of foreign material, however, device reprocessing could not be completed due to the clogged nozzle.The issue was likely the result of user handling/mishandling.The issue may be detected/prevented by following the instructions for use sections below: chapter 7cleaning, disinfection, and sterilization procedures chapter 8 reprocessing workflow for endoscopes and accessories chapter 9 reprocessing the endoscope(and related reprocessing accessories) olympus will continue to monitor field performance for this device.
