It was reported that filter break occurred.A transcatheter aortic valve replacement procedure was being performed and sentinel cerebral protection system (cps) was selected for use.The patient's anatomy contained moderate to severe calcified arteries and a tortuous right subclavian artery.The physician decided to pre-wire into the ascending aorta using a long, non-boston scientific (bsc) 0.014mm guidewire.The sentinel cps was advanced over the non-bsc guidewire.Difficulty was encountered in obtaining proper position and the proximal filter of the sentinel cps was deployed three or four times.The proximal filter then failed to deploy, so the sentinel cps was removed from the patient's anatomy.Once outside of the patient, an attempt was made to deploy the proximal filter, which deployed successfully.However, upon inspection, the proximal filter wire loop had partially disconnected from the proximal filter.A new sentinel cps was successfully used to complete the procedure.No patient complications were reported.
