Event Description
It was reported that a patient underwent an atrial fibrillation ablation procedure with a thermocool® smart touch® sf bi-directional navigation catheter and during priming and after three hours from the start of catheter use (while ablating the left atrium) there was an irrigation issue.A bubble was identified in below the sensor by the pump and could not be removed.Bubble movement was present while the pump was operating, and the error was present at the pump.A stuck irrigation hole was found.Transthoracic echocardiography was used to confirm the presence of pericardial effusion.No pericardial effusion occurred.

On 24-jan-2024, the product investigation was completed as the device was not returned.Additional information received indicates that the device is not available for return, therefore no product investigation can be performed, and the customer complaint cannot be confirmed.A manufacturing record evaluation was performed for the finished device number lot 31156469l and no internal action related to the complaint was found during the review.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's reference number: (b)(4).
