This report is based on information provided by philips remote service engineer (rse) and has been investigated by the philips complaint handling team.Philips received a complaint on the tempus ls manual indicating that the device failed to pace during a training exercise with a "dpm hardware failure" message appearing.Remote service was provided and the customer requested a replacement device.A replacement device was sent out.The complaint was escalated for technical investigation.Although the initial entry tests were passed and no malfunction was detected, the log files were extracted and investigated.Error 26 appeared in the log file.The root cause could not be determined as the error was not reproducible.Based on the information available and the testing conducted, the cause of the reported problem cannot be determined however the likely cause is the board-to-board connector between the dpm board and the mainboard.The reported problem was confirmed.Based on the information available and results of additional analysis further action has been initiated.An analysis was performed by a vigilance reporting specialist (vrs).The vrs determined that while no harm was alleged, recurrence of this failure mode during clinical use could cause or contribute to death or serious injury.Therefore, this failure mode meets the definition of a reportable malfunction.The data entered in this complaint record will be utilized for product quality and safety improvements per the post market surveillance and risk management processes.The device was replaced to resolve the issue.The investigation concludes that no further action is required at this time.If additional information is received the complaint file will be reopened.
