(b)(4).This follow-up report is being submitted to relay additional information.The following sections were updated: b4, b5, d9, g3, g6, h2, h3, h6, h11.Visual examination of the returned products could not verify item and lot numbers.The pin is broken off the femur part, which is also broken in half.The bearing exhibits gouges and dents.Optical images of the fracture surface exhibited artifacts such as ratchet marks and beach marks, suggesting that the device possibly fractured due to fatigue mode of failure.A review of the device manufacturing records could not be performed due to missing lot number.Radiographs were provided and reviewed by a radiologist.The review identified that there is a medial unicompartmental arthroplasty with a fractured femoral implant seen best on the lateral view.The femoral implant is loose and slightly displaced.The tibial implant appears normal.A large joint effusion is noted.There is patellofemoral joint arthrosis.With the available information, a definitive root cause could not be determined.If any further information is found which would change or alter any conclusions or information, a supplemental report will be filed accordingly.Zimmer biomet will continue to monitor for trends.
