The device was not returned to olympus for evaluation.The customer an olympus trained biomed call in and was assisted via phone.Olympus technician troubleshooted with customer and informed the customer it could be sensors and provided part # for sensors, it could be the main board and an fse would have to come out to replace.The water was on, and customer was instructed to connect his maintenance tool to see he can identify an issue that way.Reportedly, the customer called backed and communicated to the technician that the machine fixed itself.The investigation is ongoing, and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
It was reported, the endoscope reprocessor is unable to load lcg (disinfectant solution counter).The issue occurred during preparation for use.There were no reports of patient harm.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 7 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be established.The suggested phenomenon was presumed to have been due to the water supply for diluting disinfectant being unavailable due to accidental failure of the solenoid valve to supply water.As a result, disinfectant preparation process was not completed.A further cause of the accidental failure could not be further presumed.As there was no information received of clear misuse of the device, there is no labeling advisory that was deemed required or recommended for the user facility.Olympus will continue to monitor field performance for this device.
