Event Description
Medtronic received information that during implant of this transcatheter bioprosthetic valve, a 22 millimeter (mm) non-medtronic balloon was used for pre-dilatation.The valve was deployed two times and was recaptured for repositioning.On the final deployment attempt, the valve was implanted at a depth of 4 mm below the annular plane.Left anterior oblique (lao) was used.30 seconds after the valve was implanted and the delivery catheter system (dcs) was successfully removed, the valve dislodged to the ascending aorta.It was reported the patient had a highly calcified aortic valve with an asymmetric annulus shape.A second valve was implanted in a deeper position successfully with no paravalvular leak (pvl).It was reported that the minimum axis of the annulus was 21,8 mm.No additional adverse patient effects were reported.
 
Event Description
Additional information was received that the valve was recaptured two times due to a high position of the valve.After the valve dislodged, moderate-severe regurgitation and hemodynamic compromise occurred.No additional adverse patient effects were reported.
