The device has not been received for analysis.Upon receipt and completion of the failure analysis of the complaint device, if there is any further relevant information from that review, a supplemental medwatch will be filed.
 
Event Description
It was reported that a versa cross connect access solution was selected for use during a watchman procedure.The physician attempted to use versa cross mechanical guidewire to go into superior vena cava (svc) to engage fossa.The mechanical guidewire got stuck in the versa cross dilator and got unraveled (but in one piece).The mgw got stuck trying to withdraw from dilator, thus the versa cross dilator was removed and the versa cross rf wire inserted for successful case.No patient complications were reported.The procedure was completed successfully.The device is expected to be returned for analysis.No other issues were noted.
