Event Description
It was reported, the insufflator had error reported on pressure.The issue occurred during preparation of use of a therapeutic laparoscopic procedure that was completed using a similar device were no reports of patient harm.
 
Manufacturer Narrative
The device was not returned to olympus for evaluation.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

Event Description
It was reported, the insufflator indicators on the front panel were flickering.The issue occurred during preparation of use of a therapeutic laparoscopic procedure that was completed using a similar device were no reports of patient harm.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 9 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be established.The suggested phenomenon was presumed to have been due to the suction valve not being opened due to mis-operation of the device.However, it was not possible to further identify the cause of the defect.As there was no information received of clear misuse of the device, there is no labeling advisory that was deemed required or recommended for the user facility.Olympus will continue to monitor field performance for this device.
