Event Description

According to the reporter, during a laparoscopic fundoplication, when the 2nd firing was attempted to be performed, the clip applier was stuck, it was attempted to be unblocked many times but it did not work.It was also reported that the jaws of the device would not close.Another device was used to resolve the issue.There was no patient injury.
