Event Description
It was observed that during the device evaluation, the duodenovideoscope exhibited a cracked distal end.There were no reports of patient involvement.
 
Manufacturer Narrative
The device was returned to olympus for evaluation and the evaluation found worn glue between the distal end and the server plug in, parts that needed replacement, a buckled connecting tube, a shaved forceps channel port, and an air/water cylinder and suction cylinder with no color.The following components were found scratched: switch box, right/left knob, scope connector cover unit, and the scope connector.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the reported event is unable to be determined.However, the likely cause of the reported event is due to physical stress such as hitting/dropping distal end, chemical stress from chemical solutions.Olympus will continue to monitor field performance for this device.
