Event Description
A protégé rx was intended to be used for treatment of a plaque lesion with 70% stenosis in the mid common carotid artery.Embolic protection was used.The protégé rx was prepped as per the ifu with no issues identified.The lesion was pre-dilated with a 5*30mm pre-dilation device.The device passed through a previously deployed stent.No resistance was encountered during advancement.The lock pin was checked for securement prior to the procedure and removed when the device reached the diseased site.It was reported the stent was unable to deploy.No intervention was required for removal of the device and the device was safely removed from the patient.Another product was used to complete the case.No patient injury.Stent struts were exposed on device removal.
