Event Description

Medtronic received information that during the implant of this transcatheter bioprosthetic valve (f593699), the valve was loaded and checked under fluoroscopy.As the physician attempted to insert the delivery catheter system (dcs) into the patient, they were met with resistance.The valve was never fully inserted into the patient.They removed the dcs and upon inspection, the inflow portion of the valve, the inflow portion of the valve closest to the dilator tip appeared to begin to push through the capsule material.It was reported that, "no valve material actually broke through completely, more like stretched and made thin the capsule material." the dcs was discarded.The valve appeared normal but when attempting to reload the valve into a new dcs, the valve was not behaving normally and it was decided to discard the valve.Subsequently, a new valve and dcs were used and the valve was implanted in the aortic position successfully.Of note, the patient's anatomy was small but within scope of the inline sheath and no calcium was present so the dcs should have no issues tracking through the patient.No adverse patient effects were reported.
