A facility reported that after they used the perforator id (b)(6) they noticed that the perforator label had been rubbing against the portion of the scalp that had been pulled back to allow them to prepare for the craniotomy.The tissue had blood on it and the bone was being irrigated during the procedure, so the combination of blood and irrigation eventually moistened the sticker and the friction caused by rubbing against the exposed tissue led to part of the sticker rubbing away.This did not affect the procedure.The perforator was stored on a cart in the hallway of the operating room, which is where they have stored the perforators for a while.It is unknown if any label parts fell into the surgical site.Due to the friction, it is possible that very small pieces might have, but they may have been removed with irrigation during and after the procedure.
