Event Description
It was reported that one day post implant of the cardiac resynchronization therapy pacemaker (crt-p), a device check was performed, and it was noted that the feature that monitors and auto adjust left ventricular (lv) lead threshold did not run.  an attempt to run the lv lead automated threshold pacing polarities test was performed but, none of the thresholds were reported, only lines were noted where the threshold reading should have been present.  additionally, it was reported that the effective pacing percentage was very low.  a review of the left ventricular threshold was performed and noted that the atrial event/signal was on the lv electrograms (egm) which suggested lv lead dislodge.  the crt-p and lv lead remain in use.  no patient complications have been reported as a result of this event.

Event Description
It was additionally reported that the lv lead was repositioned.The patient was later seen for a routine device check and it was observed that measurement were within normal limits.The lead is still in use.
