Event Description
|
It was reported that the procedure was to treat a lesion in the right superficial femoral artery (sfa) with heavy calcification.The emboshield nav 6 embolic protection system (eps) was used, however, the filter got stuck upon partial retrieval.The eps was removed from the sheath, however, the filter separated and a huge chunk of the calcium was in the filter.Surgical cutdown was performed to remove the filter from the anatomy.There was no adverse patient sequelae and there was no clinically significant delay in the procedure.No additional information was provided.

Visual analysis was performed on the returned product.The reported retraction problem could not be confirmed due to the condition of the returned unit.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar complaints reported from this lot.The investigation determined that the reported difficulties resulting in surgical intervention appear to be related to circumstances of the procedure.Based on the reported information, it is likely that the difficulty retrieving the filter was due to a large embolic load causing difficulty pulling the filter into the retrieval catheter resulting in damage to the filtration element requiring surgical intervention to remove the filter from the anatomy.There is no indication of a product quality issue with respect to the design, manufacture, or labeling of the emboshield nav6 embolic protection device.
