Event Description
|
It was observed that during the device evaluation, the evis exera ii duodenovideoscope exhibited a gap between the distal end and the z-block and the adhesive around the objective lens had foreign objects.There were no reports of patient involvement.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 13 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be established.The suggested phenomenon of "gap between z-block and c-body [distal end of the device]" was presumed to have been due to physical stress such as hitting/dropping distal end or chemical stress such as chemical solution used.However, it was not possible to further specify the cause of the event.The suggested phenomenon of "lens glue has foreign material" was presumed to have been due to a leakage from the device, disallowing proper reprocessing.As there was no information received of clear misuse of the device, there is no labeling advisory that was deemed required or recommended for the user facility.Olympus will continue to monitor field performance for this device.
