A physician reported a codman perforator (id 261221) disassembled from the outer plastic sleeve when pulling the product from cranium after making the fifth burr hole.It got stuck in the cranium but surgeon was able to pull it out using a reul forceps.The procedure was completed with a replacement product available.No surgical delay was reported.The procedure performed was brain tumor removal surgery, and the drill used with the perforator was midas.According to information provided, it is unknown if any part fell into the surgical site.It is unknown if the drill was electric or pneumatic.It is also unknown if the perforator clicked in place in the drill, and if the recommended spring tests were performed between each burr hole.
