Event Description
It was reported that during a recanalization procedure in the left superficial femoral artery through the right common femoral artery access, the guidewire allegedly got sheared in three different places.It was further reported that some of the larger fractured fragments of the guidewire were removed using snare.Reportedly, three smaller fragmented pieces of the guidewire allegedly remained in the patient's upper left thigh.The current status of the patient is unknown.

H10: the medical device manufacturer (d3) and manufacturing location (g1) for the straub product was selected as unknown due to system limitations.The correct medical device manufacturer and manufacturing location are straub medical us.H10: as the lot number for the device was provided, a review of the device history records is currently being performed.The return of the sample is pending.However, an image and a photo were provided for review.The investigation of the reported event is currently underway.H10: d4 (expiry date: 11/2024).H11: section a through f ¿ the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.

H10: the medical device manufacturer (d3) and manufacturing location (g1) for the straub product was selected as unknown due to system limitations.The correct medical device manufacturer and manufacturing location are straub medical us.H10: manufacturing review: a manufacturing review was conducted and there was nothing found to indicate there was a manufacturing related cause for this event.Investigation summary: a guidewire and catheter were returned for evaluation and were physically investigated.During physical investigation, the guidewire was received with uncoiled tip.The guidewire break at 109 cm from the tip of it was also observed.The test guidewire did not pass though the catheters gearwheel.After working in the water slightly lower that nominal aspiration level was achieved.No further damages were noted.Therefore, the investigation is confirmed for the reported guidewire break.A clear root cause could not be identified but a damaged guidewire represents a known inherent risk.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.H10: d4: (expiry date: 11/2024).H11: h6: (method, result, conclusion).H11: section a through f ¿ the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
