Event Description
According to the reporter, during cardiac procedures, the electrode's insulation becomes loose and came off the electrode tip.It comes off while changing tip with wet cloth and fell in the patient cavity.The electrode was cleaned using a scratch pad but not all the time.Electrode was also bent.Per the ifu (instructions for use), power settings over those amounts could lead to damage of the electrode.The blade electrodes (e1455) maximum power setting for coagulation is 35, and cut is 50: coagulation most frequently lies at 60, cut doesn¿t go above 30.A piece / part fell into patient cavity but all piece or part was retrieved.There was no patient injury.
