It was reported that a patient underwent an atrial fibrillation (afib) procedure with a thermocool® smart touch® sf bi-directional navigation catheter and a pebax damage issue occurred.Following the right atrium being mapped, the ablation catheter was removed from the body and it was observed that the area between the proximal and distal electrode, near the m3 electrode seal there appears to be a leak.The clear space area had blood inside that could be squeezed out.The catheter was replaced and the issue resolved.There was no patient consequence reported.Additional information was received.The catheter (believed to be the pebax) was improperly sealed near the m3 electrode.Blood was able to enter into the space containing the force sensor, which could then be squeezed back out through the break in the pebax seal to the electrode.The shaft was not broken.The damage did not result in wires being exposed.Damage did not result in any lifted or sharp rings.No resistance or difficulty during insertion or removal of the catheter.The catheter was not pre-shaped.The vizigo small curve sheath was used.
