Event Description

It was reported from japan that during an unspecified surgical procedure, it was observed that the battery oscillator device blade came off from the product in question due to vibration and became unclean.A spare blade was attached and taped to the fastening area to secure it in place.There was patient involvement.There were no injuries, medical intervention or prolonged hospitalization.All available information has been disclosed.If additional information should become available, a supplemental medwatch will be submitted accordingly.

This report is being submitted in pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been able to investigate or verify prior to the required reporting date.This report does not reflect a conclusion by depuy mitek or its employees that the report constitutes an admission that the device, depuy mitek, or its employees caused or contributed to the potential event described in this report.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.Device was used for treatment, not diagnosis.The actual device was returned for evaluation.The device was visually inspected and found to have a loose knob.During evaluation, it was determined that the quick coupling for saw blades could not be fully locked and came loose when the device was running.The device also failed pretest steps for general condition, check the quick coupling for saw blades, and check oscillation frequency with frequency meter.Therefore, the reported condition was confirmed.The assignable root cause of the loose turn knob was determined to be traced to device design, which has been escalated to capa.A review of the service history record indicated that the device has not been serviced for a service condition that is relevant to the current reported condition.
