Event Description
It was reported during implant procedure that the right ventricular lead did not allow the stylet to advance.The lead helix failed to extend or retract.The lead was successfully exchanged.There were no patient consequences.
 
Manufacturer Narrative
The reported events of helix mechanism issue and failure to advance stylet were confirmed.As received, a complete lead was returned in one piece with the helix found extended and clogged with blood/ tissue.Visual and x-ray examinations found the inner coil was overtorqued inside the connector region consistent with procedural damage.After cleaning, the helix was able to retract and extend by applying torque directly to the inner coil.The full helix extension length was measured within specification.The stylet insertion test could not be tested due to overtorqued inner coil consistent with procedural damage.The cause of the reported events was isolated to the blood/ tissue and overtorque of the inner coil.Electrical testing did not find any indication of conductor fractures or internal shorts.
