Event Description

It was reported during a transurethral lithotomy (tul), targeting stones in the kidney and ureter, an ncircle tipless stone extractor's handle was damaged, and the basket would not open/close properly.The basket was used several times during the stone retrieval procedure before the issue was noted.The procedure was completed using another same type device.A section of the device did not remain inside the patient.The patient did not require any additional procedures or experience any adverse effects due to the occurrence.
