The device was returned and the evaluation found that there was light-emitting diode (led) failure on the front panel.Should additional relevant information become available, a supplemental report will be submitted.
 
Event Description
It was reported, the insufflator continued to insufflate and would not alarm when the pressure became too high, the patient was insufflated too much.The issue occurred during an unspecified procedure.The procedure was completed.There were no reports of patient injury or harm.

Additional findings noted the unit continues to insufflate when pressure is too high.Unable to confirm the initial report previously stated in section b5.Unit continues to insufflate and does not alarm when pressure is too high.Unable to confirm the initial report in section b5.Additional findings noted an led failure on the front panel due to the faulty front panel.Should additional relevant information become available, a supplemental report will be submitted.

This report is being supplemented to provide additional information based on the legal manufacturer's investigation and correction to h4.A review of the device history record found no deviations that could have caused or contributed to the reported issue.The device was returned to olympus and evaluated, with no confirmation of the issue.Based on the results of the investigation, the high insufflate with no alarm was not confirmed.Olympus will continue to monitor field performance for this device.
