Event Description
The customer reported to olympus the insufflation unit kept resetting itself during an unspecified procedure.The procedure outcome was not reported.There was no report of patient injury or medical intervention associated with this event.

The device was returned to olympus and evaluated.The customer¿s allegation was not confirmed.The investigation is ongoing and follow-up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the investigation, the reported issue (device resetting itself) was not reproduced.Therefore, the root cause could not be determined.This supplemental report includes additional information received from the customer.B5 updated accordingly.Also, a correction has been made to d4 and g2 to provide information that was inadvertently not included in the initial medwatch.Olympus will continue to monitor field performance for this device.
 
Event Description
It was reported that the incident occurred during the middle of a procedure to diagnose an ectopic pregnancy.There were no error messages displayed at the time of the event.It was confirmed that the device was inspected prior to use per the olympus instructions for use (ifu) manual.Although the incident resulted in a 5-minute delay, the procedure was completed using the same device.The patient did not require any additional medical intervention and was not impacted by the event.
