The device was returned and the evaluation and the fault was not reproduced.Should additional relevant information become available, a supplemental report will be submitted.
 
Event Description
It was reported, the insufflator when set to 10mmhg did not increase the abdominal pressure.As a result, the pneumoperitoneum pressure dropped and remained at 0mmhg.The physician restarted the air delivery button and the procedure continued with no impact to the patient.The reported event resulted in a 2 minute procedural delay.The issue occurred during the therapeutic laparoscopic cholecystectomy procedure.There were no reports of patient harm.

Correction to h6 "type of investigation" code from "4114 - device not returned" to "10 - testing of actual/suspected device".This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a root cause could not be identified.However, unstable pressure levels may have been due to issues with connecting with trockers or combination devices during insufflation (air leakage) or because the patient's cavity and target area during procedure are smaller.Olympus will continue to monitor field performance for this device.
