Event Description
It was observed that during the device evaluation, the duodenovideoscope exhibited the distal end and the server plug-in (z-block) had a crack.There were no reports of patient involvement.

The device was returned and the evaluation found no additional reportable malfunctions other than those mentioned in b5.Should additional relevant information become available, a supplemental report will be submitted.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation the crack in the glue on distal end of device was confirmed.The cause of the crack in the glue was attributed to physical stress by hitting/dropping the distal end or chemical stress from chemical agents.Olympus will continue to monitor field performance for this device.
