It was reported that unspecified bd infusion set had valve malfunction the following information was received by the initial reporter with the : rcc received a complaint via email.Email(s) attached.User started secondary medication (ceftriaxone) on patient through alaris pump.When looked at chamber, seemed to be running too fast (drip rate looked fast).Paused the channel medication was running through but noticed drips in chamber kept going.Clamped medication and switched medication to different channel on the same alaris pump brain.Started medication in new channel but witnessed same fast drip rate.Pump gave red alarm "channel error".Medication clamped, disconnected from patient, and removed from pump.¿ i had a video call with (b)(6) this afternoon and was able to confirm that this is a check valve failure.Took a picture of the primary drip chamber which was full.The medication is yellow and you can see discoloration in the primary bag.
