It was reported that during an appendectomy procedure, the tip of the device was strange, as if it were between crooked and broken.Clips didn't come out well.Another device was used to complete the procedure.Procedure completed successfully.No patient consequences.

(b)(4).Date sent: 5/15/2024.D4: batch # a9e78y.Investigation summary: the product was returned to ethicon for evaluation.Visual inspection and functional testing were conducted on the returned device.Visual analysis of the returned sample determined that the el5ml device was returned with the cam damaged; this condition would not allow the jaws to collapse in order to form the clips.No functional testing could be performed due to the returned condition of the device.In order to evaluate the condition of the internal components of the device, it was disassembled.Upon disassembling, no anomalies were found.Two(2) clips were found inside clip track.Although no conclusion could be reach on the cause of the reported event, the instructions for use do contain the following caution: firing the instrument over another clip or instrument can also damage a properly deployed clip, disrupt related vessels and structures, and damage the instrument.Avoid firing the instrument over another clip or instrument.Firing the instrument in this manner may distort or yield the instrument jaws, which can cause the instrument to release the clip prematurely.A manufacturing record evaluation was performed for the finished device batch and lot number, and no non-conformances were identified.
