Event Description
It was reported that b30 and e216 scope communication errors were observed using the light source.The issue occurred during an unspecified procedure.There were no reports of patient harm.
 
Manufacturer Narrative
The device was not returned.Troubleshooting was performed advising to power off both the cv-190 and clv-190, remove the scope, clean the contact points on the scope with an alcohol preparation pad, wait for the scope to dry, reconnect the scope, power everything back up and observe the result.In addition, it was advised to always disconnect or connect the scope when the light source is powered off if b30 or e216 errors are present.Both the cv-190 and clv-190 must be powered off.Should additional relevant information become available, a supplemental report will be submitted.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.New information added to the following field: d9, h3, and h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over nine (9) years since the subject device was manufactured.The device was not returned to olympus; instead, the customer requested a visit from a field service engineer (fse) to the facility.The fse reported that the device was inspected, and no issues were verified.Based on the results of the investigation, a definitive root cause could not be determined.Olympus will continue to monitor the field performance of this device.
