Event Description
|
According to the reporter, during a laparoscopic cholecystectomy procedure, when placing the clip on the cystic artery, the surgeon was able to press the handle when first inserting the clip, and then it was no longer possible to reload the clip.A new device was used to resolve the issue.There was no patient injury.
