It was reported that during reprocessing, the cysto-nephro videoscope pressure cap was not attached when blowing the scope.There were no reports of patient harm.

This report is being supplemented to provide additional information based on the device evaluation and the legal manufacturer's final investigation.New information added to the following fields: a review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over (14) years since the subject device was manufactured.Based on the results of the investigation, it is not possible to establish the root cause.The event can be detected and prevented in accordance with the following instructions for use: instructions.Visera cysto-nephro videoscope.Olympus cyf type v2.Chapter 5 reprocessing: general policy.Chapter 6 compatible reprocessing methods and chemical agents.Chapter 7 cleaning, disinfection, and sterilization procedures.Olympus will continue to monitor field performance for this device.
