The healthcare professional reported that during an endovascular embolization procedure, the 4 mm x 39 mm enterprise®2 stent (encr403912 / 8302865) was impeded in the y-connector and could not advance further.The physician retracted the stent for inspection and observed that the stent body was prematurely separated from the delivery wire.The physician replaced the stent to complete the procedure using the original concomitant microcatheter.There was no report of any negative patient impact.The complaint device is reported as not available for return.On 13-mar-2024, additional information was received.The information indicated that the target vessel was the ophthalmic segment of the left internal carotid artery.The microcatheter used was a prowler select plus; continuous flush was maintained through the microcatheter.The replacement stent was another 4 mm x 39 mm enterprise®2 stent (encr403912).There was no procedure delay as a result of the reported issue.
