It was reported that this cardiac resynchronization therapy defibrillator (crt-d) delivered inappropriate anti-tachycardia pacing and three shocks due to an episode of atrial driven arrhythmia with possible rapid ventricular response.This crt-d remains in service.No adverse patient effects were reported.Approximately two years later, this patient received another shock.A review by technical services (ts) noted that it also appeared to be a result of atrial arrhythmia with rapid ventricular response (rvr).The electrophysiology (ep) specialist agreed with the assessment.It was further reported that the patient experienced a syncope that coincided with the arrhythmia events.This device remains in service.No additional adverse patient effects were reported.
