(b)(6) h.6.Investigation summary: bd did not receive samples or photos for review and analysis in support of this complaint.Additionally, 30 retain samples were subjected to a visual inspection for stopper cocked.0 of 30 samples failed.Therefore, bd is unable to confirm the customer reported defect based on retain sample analysis.The device history record was reviewed with no issues being identified.There were no quality notifications.All process and final inspections comply with specification requirements.Based on a review of batch records and the investigation results, no root cause from the manufacturing process has been identified as a contributor to the customer reported defect.Complaints received for this device and reported condition will continue to be tracked and trended.Information will be captured on trend reports and monitored.Our business team regularly reviews the collected data for identification of emerging trends.H3 other text : see h10.
 
Event Description
It was reported that the bd vacutainer® k2 edta (k2e) 5.4mg blood collection tubes had a stopper that was difficult to remove.This affected 12 tubes.There was no report of patient impact.
