The serial number of the customer's accu-chek inform ii meter is (b)(6).The customer stated that the qc levels 1 and 2 passed on the date of event.The test strips were requested for investigation but have not been received at this time.If the product is returned in the future, a follow-up report will be submitted.On a regular basis, accu-chek inform strips of lots currently valid in the market are tested as part of routine retention testing and results have passed the internal inspection.

We received an allegation of a questionable glucose result for one patient tested with the accu-chek inform ii meter.The meter result at 8:40 am was 35 mg/dl.The patient was treated based on this result.The laboratory result at 8:53 am was 55 mg/dl.

The meter was received for investigation.The meter was tested using retention test strips and controls.Investigation results: control range: level 1 - 30-60 mg/dl.Results: level 1 ¿ 44, 43, 44 mg/dl.Control range: level 2 261-353 mg/dl.Results: level 2 ¿ 301, 305, 307 mg/dl.The investigation results are within acceptable range.The investigation did not identify a product problem.
